COCOMO-studiet

Copenhagen comorbidity in HIV infection (COCOMO) studiet.

​​

Formålet med COCOMO-studiet er at undersøge forekomsten af anden samtidig sygdom (infektiøs og ikke-infektiøs komorbiditet) blandt personer, der lever med HIV og bor i Københavnsområdet. 

Studiet blev igangsat i 2015, og 1099 personer med HIV fra Hvidovre Hospital og Rigshospitalet valgte at deltage. Alle deltagere svarede på et spørgeskema og fik lavet et helbredstjek med bl.a. blodprøver, pusteprøve (spirometri) og skanning af hjerte og lunger (CT). Siden har omkring 90 % af deltagerne været med i en opfølgende helbredsundersøgelse i 2017-2018 med mulighed for at få foretaget ultralydsscanning (ekkokardiografi) af hjertet.

​Til dato er 49 artikler med data fra COCOMO-studiet blevet publiceret i internationale fagfællebedømte tidsskrifter. Et udpluk af hovedfundene kan ses nedenfor. 

Hjertesygdom

Flere personer med HIV får med alderen forandringer på deres ekg, har højere forekomst af åreforkalkningssygdom i benene, forhøjet blodtryk i lungerne, forstørret hjertemuskel og højere risiko for udposning på hovedpulsåren. Alt sammen tegn på begyndende eller manifest hjertekarsygdom. ​​

Lungesygdom

Personer med HIV har en højere forekomst af kronisk obstruktiv lungesygdom (KOL) end HIV-negative personer. Risikofaktorerne for at udvikle KOL er de samme som i baggrundsbefolkningen, men rygning ser ud til have en større skadelig effekt på lungerne hos personer med HIV end hos HIV-negative. Der er ikke tegn på, at personer med HIV er i øget risiko for at have beskadiget lungevæv (emfysem) eller lungetumorer.

Lever- og nyresygdom 

Vi har fundet, at personer med HIV har højere forekomst af leverfibrose (arvæv) end HIV-negative personer, mens forekomst af fedtlever er lavere. Sandsynligheden for at have nedsat nyrefunktion var mere end tre gange højere for personer med HIV sammenlignet med HIV-negative personer.

Metabolisme

Personer med HIV har højere forekomst af abdominal fedme og metabolisk sygdom, som er velkendte risikofaktorer for kardiovaskulære sygdomme. Vi har fundet, at tidligere anvendte HIV-behandlinger er forbundet med mere fedt omkring tarmene, selv mange år efter behandlingen er stoppet. Opstart af nyere behandling medfører først vægtøgning, men effekten aftager over tid, og langtidsbrug medfører ikke øget vægtøgning.

Hormonsygdomme og blodsygdomme 

Vi har fundet, at personer med HIV har højere forekomst af sukkersyge end HIV-negative personer. Der er ingen tegn på, at stofskiftesygdomme skulle være mere hyppige hos personer med HIV, end sammenlignelige HIV-negative personer, men flere har blodmangel og/eller forstyrrelser i blodets evne til at størkne. Knogleskørhed findes hyppigere hos personer med HIV end hos HIV-negative personer, men når man tager højde for risikofaktorer såsom rygning, er der ingen forskel i knogleskørhed.

Inflammation og tryptofanmetabolisme

Vi har fundet, at forstyrret tryptofanmetabolisme er forbundet med fedme, depression og ændringer i tarmmikrobiom.
