Please read them thoroughly through and adjust according to your national/local guidelines prior to applying for ethical approval.
In the bottom of this page you will a document named "Information provided to the data subject, to ensure transparency of regulation (EU) 2016/679 (GDPR)". This is a document that needs to be attached, as an appendix, to the consent form. Within the document, you can fill in your site's name, under point 7.
